Sonographic Differential Diagnosis
Given the large size of the mass, lack of reactive adjacent tissues, vague clinical history, and minimal changes in laboratory values, a benign or low-grade malignant process is suspected primarily, and consideration is given to a hepatoma or giant hepatocellular carcinoma. An aggressive neoplastic process or chronic inflammatory or granulomatous pathology cannot be ruled out definitively but is considered less likely.
Image Interpretation
A large, echogenic, mildly heteroechoic, fairly discretely marginated, ovoid mass presents within the liver adjacent to the gallbladder and portal hilus. The mass appears to be located within the caudal aspects of the quadrate and left middle liver lobes. Despite its medial location in intimate association with the portal hilus, involvement or impingement on the regional biliary structures does not appear to be present; that is, lack of periportal lymph node enlargement, normal-appearing gallbladder, lack of common bile duct distension.
Outcome
The recovery was uneventful. A dental procedure was performed at a later date, after which the hyperglobulinemia resolved. The patient was examined and showed good body condition without clinical issues 1 year post-diagnosis.
