Pyloric foreign body and gastric lymphoplasmacytic inflammation in a 15 year old DSH cat

History

A 15-year-old DSH presented for projectile vomiting, primarily 2-4 hours postprandial, with a decreased appetite and minor weight loss. A CBC and blood chemistry profile revealed mild azotemia but were otherwise normal.

Clinical Differential Diagnosis

Gastritis, neoplasia, IBD, foreign body, motility disorder.

DX

Pyloric hair foreign body, mild lymphoplasmacytic gastric inflammation, lymphocytic lymphoma

Sampling

Exploratory surgery revealed mild hair accumulation in the stomach with mild lymphoplasmacytic gastric inflammation. Concurrently, a distal small-intestine thickening was biopsied and histopathology revealed lymphocytic lymphoma. This portion of the intestine was not found during the sonogram.

Sonographic Differential Diagnosis

The differential diagnosis included a partially or intermittently obstructing pyloric foreign body. The sonographic appearance supported a soft structure such as cloth or hair.

Image Interpretation

The pylorus was focally, mildly distended with echogenic ingesta. Fragmented dirty distal acoustic shadowing was associated with the pyloric structure, suggesting the presence of soft material. The pyloric wall appeared to possess a normal, uniform wall thickness. The adjacent soft tissue structures were within normal limits.

Outcome

This patient responded well over 6 months on a prednisone/chlorambucil combination protocol.
