Left ureteral calculus with obstruction in a 4 year old FS DSH cat

History

 A 4-year-old female spayed DSH cat was presented for a follow-up evaluation of left-sided hydronephrosis with left ureterolith. Abnormalities on laboratory testing included azotemia and mild anemia.

Clinical Differential Diagnosis

Ureterolith with secondary hydronephrosis. Chronic kidney disease.

DX

Left ureteral obstruction with progressive hydronephrosis and nephritis.

Sonographic Differential Diagnosis

Progressive hydronephrosis and nephritis of the left kidney was noted. Left ureteral obstruction was still evident, and the calculus has not moved, perhaps due to the regional thickening of the ureter. The ureteral contour was also changed and has become more conical and dilated. This is indicative of a further progressive obstructive pattern. No peristalsis of the ureter was noted during the exam, which is a negative prognostic indicator. Sonographically, the right kidney appeared within normal limits with the exception of minor degenerative changes. This may be indicative of the ability of the right kidney to support the patient metabolically.

Image Interpretation

Slight bladder sand was noted in an otherwise normal bladder. The left ureteral calculus was embedded within the ureter. The calculus measured 0.45 cm. The ureter was now dilated at 0.42 cm. The ureter has thickened and has increased in echogenicity. The calculus appeared to be 2.2 cm distal from the left renal pelvis. Hydronephrosis is moderate at this point and appears to be progressive. Pyelectasia was noted, and measured approximately 2 x 1.5 cm. A pericapsular inflammatory pattern was persistent on the left kidney. The right kidney appeared unremarkable with a minor interstitial nephrosis pattern.
