RAD – Mild left-sided cardiomegaly likely from mitral regurgitation without congestive heart failure (CHF) and no evidence of metastases in an intact 7 year old F Scottish Terrier dog

History

This 7 year old intact F Scottis Terrier dog presented for on and off hematuria for weeks, 1/6 systolic murmur, 2 small firm mammary nodules, heavy tartar/gingivitis. Please comment on L3 vertebrae.

DX

Suspect mild leftsided cardiomegaly most likely owing to mitral regurgitation without evidence of congestive heart failure.

Image Interpretation

Rads of the thorax and abdomen – There is mild elongation of the cardiac silhouette as well as mild left atrial tenting with partial loss of the caudal cardiac waist indicating left atrial enlargement.
There is no evidence of pulmonary venous congestion or cardiogenic pulmonary edema at this stage.
The lung reveals a moderate generalized bronchointerstitial pattern which is likely to be age related. There is no evidence of metastatic spread to the lungs. No mediastinal lymph node enlargement is noted.
The serosal detail is within normal limits. The liver and spleen are within normal limits regarding their size, shape and opacity.
Small mineral opacities are superimposed onto the region of the kidney but these are likely artefactual as similar opacities are seen throughout all images.
The urinary bladder neck appears to be relatively wide, which may be caused by actual widening or summation with the cervix/uterine body.
There is no evidence of regional lymph node enlargement within the hypogastrium.
The ventral contour of the vertebral body of L3 is mildly ill defined and reduced in opacity which is a common incidental finding associated with the attachment oft he diaphragmatic crurae to the cranial lumbar vertebrae. There is no evidence of aggressive ostelysis.
The expected age related changes are seen throughout the axial and appendicular sceleton including spondyloses, spondylarthroses, mineralized intervertebral discs as well as mild bilateral shoulder joint osteoarthritis. There is no evidence of cuurent clinical significance of these changes.

Outcome

The lungs and mediastinal lymph nodes are negative for metastatic spread. The bronchointerstitial lung pattern is either age related or the consequence of a previous bronchitis.

Possible widening of the urinary bladder neck.

Expected age related changes of the skeleton.

A full abdominal ultrasound and cardiac echo are strongly recommended to further define the underlying cause of the hematuria and radiographic findings as mentioned above.
