DX

inflamed gallbladder mucocele, surgical emergency

Image Interpretation

The liver images from right and left intercostal as well as subcostal views revealed subjectively normal liver size, contour, and structure. Parenchymal echogenicity was naturally coarse and hypoechoic to the spleen. The common bile duct appeared dilated at 0.8 cm with echogenic debris. This is consistent with mucoduct. The gallbladder wall was double layered. The gallbladder in this patient presented severe dilation along with suspended, striating bile. Regional pericapsular inflammation was noted. All aspects of inflamed gallbladder mucocele are present. The pancreas was largely unremarkable with minor extension of inflammation noted associated with the gallbladder

Outcome

Dilated common bile duct with echogenic debris. This is consistent with mucoduct.
Common bile duct lavage is necessary.
