Congenital Addison’s disease in a 5 month old M English Springer Spaniel

History

 

History:  A 5-month-old male English springer spaniel was presented for evaluation of acute onset diarrhea and vomiting. On physical examination lethargy, dehydration, hypothermia, and bradycardia was present. Abnormalities on CBC and serum biochemistry were mild lymphocytosis and mild eosinophilia, azotemia, hyperphosphatemia, hypercalcemia, hyponatremia and hypokalemia.

Urinalysis showed SG of SG-1.015, proteinuria, and glycosuria.

Clinical Differential Diagnosis

Addison’s disease
GI disease – foreign body, infectious (viral, bacterial, helminths)
Renal – acute kidney injury, renal dysplasia

DX

Flattened adrenal glands. The profile is consistent with congenital Addison’s. Slight renal pylectasia

Image Interpretation

The left adrenal gland was flattened in contour and subnormal in width measuring 1.55 x 0.31 cm at the cranial pole and 0.26 cm at the caudal pole. The right adrenal gland was also flattened and measured 0.35 cm in width. 

Outcome

urine culture and sensitivity as well as treatment for Addison’s disease is recommended
