History

A 6-month-old intact male Labrador was presented for evaluation of diarrhea and vomiting for approximately 1 week and more recently only hemorrhagic diarrhea.

Clinical Differential Diagnosis

Infectious (viral, bacterial, helminths, protozoa), foreign body, intussusception, dietary hypersensitivity

Sonographic Differential Diagnosis

Intussusception with regional lymphadenopathy. Likely enteritis, potential underlying parasitic disease. The lymph nodes met neoplastic criteria; however, this was not overtly suspected. Culture and biopsy of the mesenteric root lymph nodes would be recommended at the time of surgery. Guarded prognosis depending upon underlying histopathology.

Image Interpretation

The ileocecal junction revealed a large intussusception that measured approximately 6.0 cm and appeared to involve the mesenteric artery. An obstructive pattern was noted with echogenic chyme within the stomach continuing into the small intestine. This may be able to be manually reduced without resection. The mesenteric root lymph nodes were enlarged in this patient, hypoechoic and irregular measuring 3.86 x 2.15 cm with reactive surrounding fat. Regional lymphadenopathy should be sampled at the time of surgery.
