History

A 13-year-old FS mixed breed canine was presented for weakness and anorexia. On physical examination the mucus membranes were pale and the abdomen was tense on palpation. Urinalysis showed an inappropriate SG, proteinuria, pyuria, and 2+ granular casts. Abnormalities on CBC and serum biochemistry were anemia, leukocytosis, neutrophilia, monocytosis, hyperglobulinemia, azotemia, and elevated ALP activity, cholesterol and amylase. T4 was low.

Renal – pyelonephritis, bacterial nephritis, abscess, neoplasia with secondary infection Peritonitis – bacterial, urine, bile Pancreas – pancreatitis, neoplasia, abscess Spleen – neoplasia, torsion, splenitis

Sonographic Differential Diagnosis

Multiple splenic masses. Right liver mass with similar echogenicity, which is likely a metastatic lesion. Normal heart.

Image Interpretation

The liver was largely normal except for a deep right liver mass that measured 6.9 cm. The splenic presented multiple, mixed, hypoechoic parenchymal masses and microcavitated masses with reactive omentum. This is suggestive for multi focal sarcoma or similar. The splenic masses measured 15 and 9 cm with other variable nodular changes. Rapid evaluation of the right heart revealed no evidence of metastatic disease.

Outcome

The pet was referred to an oncologist. One month after the ultrasound the owner elected to humanely euthanize the pet.
