Clinical Differential Diagnosis

Hyperthyroidism Cardiac disease: cardiomyopathy (dilated/hypertrophic), valvular disease, myocarditis, hypertrophy secondary to hypertension.

Unclassified cardiomyopathy, Left and right sided CHF

Sonographic Differential Diagnosis

Unclassified cardiomyopathy, DCM type presentation. Left and right sided congestive heart failure and pleural effusion.

Image Interpretation

The cardiac presentation in this patient presented significant left atrial and right atrial enlargement with subnormal contractility and left ventricular internal diameter dilation. Septal free wall thicknesses were mildly excessive with myocardial insufficiency present. This is consistent with unclassified cardiomyopathy or potentially dilated cardiomyopathy with left and right sided congestive heart failure. A mild amount of pleural effusion was noted. Outflow velocities were subnormal in the aortic and pulmonic outflow tract. Mitral insufficiency was significant at 4.25 m/sec.

Outcome

Taurine supplementation would be ideal in this patient. Lasix at 2-3 mg/kg b.i.d., ace inhibitor at 0.5 mg/kg s.i.d. and off label use of Pimobendan at 0.25 mg/kg b.i.d. would also be indicated. Aspirin therapy can also be considered. A recheck echocardiogram is recommended in one month. Guarded long term prognosis.
