Colonic obstipation in a 7 year old MN DSH cat

History

A 7-year-old MN DSH cat was presented for intermittent vomiting typified by vomiting 2-3 times a week. The patient also had a history of constipation. The physical exam revealed mild dehydration (estimated at 5%), pyrexia, a grade IV/VI heart murmur and large amounts of firm stool in the descending colon. CBC showed a neutrophilia. Blood chemistry showed elevated ALT and hyperamylasemia. T4 was within normal limits.

Clinical Differential Diagnosis

Liver pathology (hepatitis, cholangiohepatitis); GI pathology ( IBD, constipation due to decreased motility, megacolon, mass lesion); gastrointestinal, hepatic or pancreatic neoplasia (lymphoma, adenocarcinoma, leiomyoma, leiomyosarcoma, mast cell tumor.)

Sonographic Differential Diagnosis

Colonic obstipation.

Image Interpretation

Significant repletion of the colon is present with hard shadowing stool. No evidence of obstructive disease is present and the colonic shadowing material is uniformly dilating the colon into the pelvic inlet.

Outcome

A subtotal colectomy was recommended. However, the owner elected to euthanize the cat.
