Splenic hematoma in a 11 year old MN Rhodesian Ridgeback dog

History

An 11 year old, MN, Rhodesian Ridgeback dog was presented for nonspecific symptoms (ADR). Abdominal mass was palpated. The dog had a subnormal temp (98.8) and pale mucus membranes. Altered CBC/Chem/UA values included leukocytosis, mild azotemia, and mild hypoglobulinemia. Radiographs demonstrated a mass effect in the right abdomen.

Comments

After ultrasound, exploratory surgery was recommended, with splenectomy and inspection of the liver (even though no evidence of metastatic disease was noted). Three view chest radiographs would be warranted prior to surgery.

Clinical Differential Diagnosis

Abdominal mass – neoplasia, cyst, granuloma or abscess of any of the following organs: liver, spleen, kidney, lymph node, mesentery, or pancreas. Splenic torsion. Hydronephrosis.

DX

Splenic hematoma

Sampling

At splenectomy, no metastasis to the liver was found. Histopathology results suggested the mass was a hematoma.

Sonographic Differential Diagnosis

Splenic mass with separate nodule. Sarcoma (round cell neoplasia), hematoma, hemangiosarcoma suspected.

Image Interpretation

The spleen presented a 6+ cm, mixed, hypoechoic mass with a separate target type nodule that measured 2.15 cm with capsular expansion. There was no evidence of rupture noted.
Rapid view of the right atrium and pericardium revealed no evidence of metastatic disease.
