Cranial mediastinal mass


  • 8 year old MN Old English Sheepdog with hypercalcaemia and inappetance
  • Pleural effusion with echogenic cranial mediastinal mass and two smaller masses (LNs) cranially?
  • What is your impression of this mass, all lymph nodes with suspect lymphoma?
  • I have taken aspirates of the larger mass and the confluent smaller more cranial mass also

    Most likely DDs are

    Hi!

    Most likely DDs are lymphoma and thymoma.

    Unfortunately, FNAs are frequently inconclusive with these and core biopsies are then required.

    A different approach is to administer Vincristine – if the mass decreases in size, it’s lymphoma, otherwise thymoma. Yet, I’m aware that this is not a scientifically correct approach 🙂

  2. The mass appears to be cystic

    The mass appears to be cystic in places . Not sure as you are scanning through it so fast. Scan slowly and try a linear probe. If it is cystic then it is a thymoma.
