Factors affecting heart position


Hi!

I’m scheduled to echo on an obese (40lb) King Charles Spaniel next week… new heart murmur, looking for MVD.

Just curious… if L sided heart enlargement rotates the heart in a certain direction, how does being extremely overweight change the heart position?  Dorsally perhaps?

And do CKCS do LA enlargement differently?  I vaguely recall hearing that…

    fat deposition shouldnt rotate the heart you just have to lower the frequency so a 40# obese dog think of it as a 60# dog. 4-5 mHz or similar frequency on phased array is where i would start to adequately penetrate the chest. Go by LA max in king charles and heart base la/ao next but june boon LA/AO underestimates the la size in king charles. Measure it to be complete but don’t make any decisions on june boon la/ao in this breed.

    When I have to do an echo on a fat little dog with excessive panting, Dr. Modler once recommended that I increase dynamic compression and reduce dynamic range.  This seems to help.
