The relatively uniform, hypoechoic thickening of the stomach wall with loss of wall layering along with regional hypoechoic, swollen lymphadenopathy makes me think lymphoma over carcinoma or other neoplasms. The small intestine appears to be affected as well with decreased motility. Neoplasia is definitely the primary rule out.
The gastric wall looks to be about 2 cm depth in the picture. I would try to FNA the gastric wall or the lymph node.
