Very painful on ultrasound of kidneys and difficult to get good images
Full urinalysis and lepto PCR pending
How would you describe these kidneys and what would your differentials be please?
6 year old female entire Labrador 30kg
Lethargy, inappetance, pu/pd
Not up to date with vaccinations

How would you describe these kidneys and what would your differentials be please?

Hyperechoic, irregular
Hyperechoic, irregular capsule, loss of cortico-medullary differentiation, pylectasia: end stage renal disease. Possible etiologies – congenital renal disease, prior toxins/hypovolumia, Leptospira, Lyme, bacterial nephritis. Biopsy at this stage often non-diagnostic. Woud culture urine as there may be a secondary UTI which is complicating the picture. Manage as for chronic kidney disease – renal diet, phosphate binders, gastric protectants, EPO as needed.
