11 year old FS maltese with intermittent yearly episodes of abdominal pain and vomiting
Vomited brown fluid during abdominal ultrasound
Bloodwork shows increased BUN=31, normal liver values, normal Tbil, and normal WBC ct.
Abdominal US shows small adrenal glands, small, degenerative kidneys, gastritis stomach pattern (retained gastric fluid and thickened wall), and both swirling, suspended debris and gravity dependent debris in the gallbladder

I am recommending treatment for gastritis, screening for Addison’s, urine culture, but am also wondering if the gallbladder could also be a player here.

Gall bladder unlikley to be a
Gall bladder unlikley to be a player as there is no elevation in liver enzyme activity, bilirubin, and white cell count. Your work up is spot on but would consider gastroscopy as well.

I agree with Remo even though
I agree with Remo even though our research shows on very RARE occasion surgical gb disease can have no liver enzyme elevation (ECVIM 2009 – http://sonopath.com/resources/research-publications) that is not the case here just a bit of sludge not likely clinical.
