Stage 3 Chronic Kidney Disease


Lucy is a 7 year old Golden retriever Fe/spayed. Been feeling fine. Routine blood work indicated that Lucy is in IRIS Stage 3 chronic kidney disease/proteinuric. Blood pressure not taken.

Lab results:

Amylase: 1512

Creat: 2.6

BUN: 31

SDMA: 23 (<14)

Urine: cysto

SG 1.017

pH 7.0

Dipstick:

1+ blood

4+ protein

Sed: 

RBC 15-20/HPF

WBC 0-2/HPF  – No inflammatory component

Bacteria none seen  (urine not cultured)

Urine Prot/Creat (done later- clean stick- no blood) 3.8

Negative for Borrelia. No titer to check for Lepto

I am posting a few cine’s and a couple of stills photos. I was wondering if I could get an opinion on the kidneys.

Bladder was clean

Rest of abdominal scan looked normal

    This is your garden variety PLN and the type of kidney we see in NJ with Lyme nephritis though negative for you but many things can do this. GN, immune mediated (lupus or other), any tick borne disease, amyloid, lepto…. Remember just a note that IRIS staging is done in a hydrated patient which Im assuming he was. Needs a renal bx or if chasing amyloid you may get an indication with an fna and rhodanine stain in labs that do it. the cortices are a kittle “puffy” and thick and a nebulous CM definition but otherwise not a big deal but azotemia isosthenuria and porteinuria.
